2015-16 Kindergarten & Head Start Registration Now Open

Will your child turn three, four or five by September 1, 2015?

If so, it’s time to register for 4 and 5–year-old kindergarten or Head Start! Families can register during school hours at any Green Bay Area Public School District school, or in the District Office, 200 South Broadway, between the hours of 7 a.m. – 5 p.m. Monday through Friday (and Wednesday until 6 p.m.). Families with children already enrolled in 4-year-old kindergarten, 5-year-old kindergarten or Head Start do not need to re-register.

Parents can complete the Head Start and/or 4K enrollment forms in advance. Forms can be picked up at the school, at the District Office, and can be found on the district’s website at http://www.gbaps.org/Parents/Pages/Forms.aspx in both English and Spanish.

What to bring to register:
• Enrollment Forms if completed prior to visit.
• A birth certificate or other verification of birth.
• Child’s immunization record.
• Verification of address.
• Medical evaluation (4K optional 5K required).
• Head Start registration requires a view of the past 12 months of income for families that apply. Income Taxes or W-2’s work well.
